A second woman has been confirmed dead following a head-on crash near Taupo on Saturday.

Taupo Police said a Suzuki Swift crossed the centre line on State Highway 1 at Five Mile Bay about 2.56pm, colliding with a Toyota RAV4.

The Suzuki driver, a 79-year-old woman, died at the scene and a 53-year-old woman who had been travelling in the Toyota's front passenger seat was taken to Taupo Hospital and died a short time later.

The Toyota's driver escaped with only minor injuries.

Police said inquiries so far suggested fatigue or a medical problem may have contributed to the crash.

Two fatal crashes have been reported so far this holiday weekend.

A third person was killed and two injured when a four-wheel-drive rolled into a river off the Maungatapu Track in the Maitai Valley, Nelson.

Emergency services were alerted to the Maitai Valley crash by one of the vehicle's occupants who ran to get help.
