An investigation into why an empty passenger train failed to stop and ran into a stop block at Auckland's Britomart station pinpoints an electrical failure in the brakes.
The collision in December 2008 caused some damage to the coupler on the front of the train and the stop block, but neither the locomotive driver or train manager on board were injured.
The Transport Accident Investigation Commission report says safety issues discussed include the speed of the train during the approach to Britomart station.
