In Iraq, elderly Father Merrin (Max von Sydow) discovers a devil figure among ancient relics.
Film star Chris MacNeil (Ellen Burstyn) is working in Washington, D.C., on a film directed by Burke Dennings (Jack MacGowran). Divorced, she maintains a residence in Georgetown with daughter Regan (Linda Blair), 12, and secretary Sharon (Kitty Winn).
A radical personality change in the young girl occurs. Dennings dies in a fall down the long flight of stairs next to MacNeil's house. When MacNeil is convinced that Regan has been possessed by the devil, she calls in Father Karras (Jason Miller) for consultation. Karras, troubled by the death of his mother (Vasiliki Maliaros) and his own faltering faith, believes that only the experienced exorcist Father Merrin can help.
Police Lieutenant Kinderman (Lee J. Cobb), a movie fan, regards Denning's death as a murder. The sickly Merrin dies in the midst of exorcising the spirits inhabiting Regan's body. Karras attacks the child, then is himself possessed before leaping to his death. Regan, apparently cured, remembers nothing.
